Please print and complete the following forms.
Space is reserved on a first-come, first-served basis. Early registration is highly recommended!
Step 1: Payment: CLICK HERE to reserve your space and pay through PayPal. Or pay by Check, maid out to the IH PTSA, Subject: Senior Party.
Step 2: Forms: REQUIRED All participants must turn in the 2020 Party Registration & Agreement (3 pages). The Emergency Care Plan is needed only if your Senior has a life-threatening medical condition.

a) In envelope labeled: PTSA: Senior Overnight Party and submit to the IHS Front Desk Receptions.
